This conference is designed for professionals devoted to transforming care through the medical home model of care, including physicians, allied health professionals, practice administrators, quality managers, and consultants.


After attending the PCMH Congress 2018 meeting, participants should be able to:

  • Outline best practices for the optimization of the PCMH within the medical neighborhood
  • Review the roles within the collaborative care environment and how these roles impact patient-centered care
  • Discuss the impact that the PCMH model of care has on patient and population health outcomes


The interactive educational conference will occur at San Diego Convention Center, San Diego, California, from September 14–16, 2018. A question-and-answer session with the faculty will follow each presentation.

To be eligible for documentation of credit, participants must attend the full activity and submit a completed evaluation form. Participants who complete the evaluation online at within four weeks of the live meeting will immediately receive documentation of credit.
